1. By submitting the form below, you hereby confirm that you are the Parent or Legal Guardian of the child that is registered to attend and participate in the Kids in Showbiz Workshop and the Kids in Showbiz TV Show (hereinafter referred to as “You” “Your” “Minor Child” “Student” “Adult”), presented by KIDS IN SHOWBIZ - RISE MEDIA BUSINESS & TECHNICAL TRAINING SCHOOL, a Maryland Nonprofit Corporation located at 12061-B Tech Road, Silver Spring, MD 20904 (hereinafter referred to as “RISE”). You (the Parent and/or Legal Guardian) Adult fully understand and agree to comply with all of the following Terms and Conditions.
2. All Parents and Students, please note: ALL KIDS IN SHOWBIZ CLASSES ARE HELD ON SUNDAYS from 1pm TO 4pm at our Silver Spring, MD location. There is also a KIDS IN SHOWBIZ AFTER SCHOOL ACADEMY which is held from 2:30pm-6:30pm Monday through Friday at the Silver Spring, MD location.
3. All Students please note that if you are absent, there are no make-up sessions. This is a Workshop and Training Module with KIDS IN SHOWBIZ - RISE MEDIA BUSINESS & TECHNICAL TRAINING SCHOOL INC., a Maryland Nonprofit Corporation.
4. Please note that all audio, video and media recordings created and produced for the Kids in Showbiz TV Show and Workshop and RISE MEDIA BUSINESS & TECHNICAL TRAINING SCHOOL Media are the sole and exclusive property in perpetuity and throughout the universe of KIDS ON SHOWBIZ - RISE MEDIA BUSINESS & TECHNICAL TRAINING SCHOOL, Inc. In addition, you hereby grant permission to RISE, to capture, videotape, film, record, photograph, digitize, edit, convert, digitally convert, upload, download, post, stream, advertise, license, promote, reproduce, broadcast, transmit, advertise, sell and solely own, copyright and control in perpetuity and throughout the universe all media containing the creative works, image and likeness of your Minor Child or Adult who is hereby registered for the KIDS IN SHOWBIZ classes. In addition, you also grant RISE and the Kids in Showbiz Workshop and TV Show the sole and exclusive right in perpetuity and throughout the universe to own, copyright, license, sell, upload, download, post, broadcast, repeat broadcast, transmit, stream, distribute, advertise, promote, digitally convert, re-record, re-edit, master, reproduce, create new Media and sell all audio, video, creative works and media that contains the image, likeness and/or creative works of Your Minor Child &/or Adult for advertising, television programming, broadcast, transmission, Internet, streaming, posting, its’ websites, social media accounts, licensing, reproduction, distribution, promotions and/or for any other commercial purposes and in doing so, you hereby warrant, grant and fully understand that RISE, it’s Websites, (&/or any of its’ Websites, Third Party licensees or business affiliates including but not limited to the Kids in Showbiz Workshop, TV Show, and the Z SUPER TELEVISION NETWORK / FIRST POWER MEDIA GROUP are not liable to compensate you (meaning the Parent and/or Minor Child or Adult) in any way, for any reason or in any form whatsoever. In addition, the Kids in Showbiz TV Workshop and TV Show, Z SUPER CHANNEL TELEVISION NETWORK, FIRST POWER MEDIA GROUP, the Kids In Showbiz Television Network and any of its’ Licensees and/or Affiliates are hereby perpetually free of any financial claims, payments, compensation, rights or royalties to be due, owed or paid to you and/or the Minor Child or Adult who is presented on Z SUPER CHANNEL TELEVISION NETWORK, the KIDS IN SHOWBIZ TELEVISION CHANNEL, RISE Websites, Social Media Accounts, any Third Party Licensees and/or the “Kids in Showbiz TV Show” (including any other form of Media (now known or in any future form) including but not limited to its’ Workshops, After School Academy, Advertising, Licensing, Promotions, Streams, Postings, Broadcast Television Programming, Social Media, Documentaries and/or its’ Websites).
5. Please note that RISE will present the Full Name of all Minor Children presented on each episode of the “Kids in Showbiz TV Show” on the Kids In Showbiz Television Network, Z Super Channel TV Network – (in the TV Episode that they are appearing in). Such Credits are to be broadcast as End of TV Show Credits.
6. The Parent and Minor Child (or Adult) are free to include the registered Child’s Name on their Child’s professional Resume (for Acting, Theater, Dance, song or Music as it pertains to your child and their participation as a Principle Actor, Dancer and/or Recording Artist on the Kids in Showbiz TV Show. The Child's Resume is for the purpose of seeking professional work in the Entertainment Industry.
7. After your Child (or Adult) has appeared on one or more episodes of the “Kids in Showbiz TV Show,” the Parent is free to register their Child (and his/her TV Show Credits) with IMDb (Internet Movie Data Base). IMDb is the Leading Information Resource for the Entertainment Industry for the purpose of hiring Cast and Crew. After your child has appeared on one or more episodes of the Kids in Showbiz TV Show, you can register for free at: https://www.imdb.com/registration/signin?ref=ft_reg.
8. Only Authorized Adults listed on this form will be allowed to pick up your child. Exceptions will only be made with verbal or written authorization from the Parent / Legal Guardian only to a Senior Staff of the Kids in Showbiz - Rise Training School.
9. Students should not knowingly come to class with a fever or flu-like illness, or when having a communicable illness (such as chicken pox, lice, ring worms, etc.). If a student arrives and is not feeling well, a parent will be notified and we will do our utmost to comfort you or your child. However, if the condition worsens, a Parent or responsible adult will be notified and a decision will be jointly made between the Parent and Company management, concerning the appropriate action that will be taken concerning an adult or child that is ill. Please note that we will not dispense medicine to any child.
10. RISE MEDIA BUSINESS & TECHNICAL TRAINING SCHOOL is a multi-use facility. It is likely that other businesses within our building may be conducting business simultaneously during our Class Programs. Unless escorted by a Company Employee, students are not permitted to play or loiter inside or outside of the facility. Likewise, students are not permitted to run, play or become loud and boisterous in the facility hallways, rooms, traffic ways, restrooms and other public spaces. Unattended students are not permitted in our private room or private office areas.
11. Smoking is not permitted in the building. The consumption of food or non-alcoholic beverages is permissible in designated areas only. Food and beverages are strictly prohibited in the recording studio and other areas where sound, lighting or other production / post –production equipment is housed.
12. Constant behavioral or disciplinary problems (fighting, disrespect to other students, Staff or Instructors, destruction of property, etc.), will not be tolerated. Any student that does not comply with facility rules will be promptly dismissed and terminated from our program and RISE reserves the right to retain all fees that have been paid. Likewise, in case of student termination from our program, any fees that are due to RISE from the Parent or Legal Guardian (or Adult) must be paid IN FULL within Two Weeks (Fourteen days) of termination.
13. RISE will not assume any responsibility, liability or financial compensation for the damage or loss of any items left at our facility.
14. Parents must review all of this entire Terms and Conditions Section with their Child (Student).
15. STUDENT NON-DISCRIMINATION POLICY: The KIDS IN SHOWBIZ / RISE MEDIA BUSINESS & TECHNICAL TRAINING SCHOOL admits students of any race, color, gender, national and ethnic origin to all the rights, privileges, programs, and activities generally accorded or made available to students at the school. It does not discriminate on the basis of race, color, gender, age, national origin, religion, sexual orientation, gender identity or expression, marital status, citizenship, disability, veteran status, or any other legally protected factor in administration of its educational policies, admissions policies, scholarship and loan programs, and athletic and other school-administered programs.
By submitting the Registration Form below, the Parent / Legal Guardian and Registered Student (or Adult) confirms to have read, understand and fully agrees to abide by all of the Terms and Conditions of the PARENTS & STUDENTS KIDS IN SHOWBIZ - RISE MEDIA BUSINESS & TECHNICAL TRAINING TERMS, CONDITIONS AND TALENT RELEASE AGREEMENT for the Z Super Channel Kids in Showbiz TV Show and the Kids in Showbiz Workshops. In addition, by submitting the Kids In Showbiz Registration Form below, the undersigned certifies that He / She is the Parent or Legal Guardian of the enrolled Minor (Child).
