By completing this incident report, it is for resolution through the Office of the Vice President and its submission initiates an investigation of the issue. The report may lead to formal charges under the Student Code of Conduct and a student conduct hearing. If you have knowledge of a student violating the Student Code of Conduct, you are encouraged to notify our office of the incident. The confidentiality of educational records is outlined by the Federal Education Rights and Privacy Act (FERPA).
